24.11.2401    IDENTIFICATION OF EMPLOYEES

(1) Each employer must ascertain the social security number of each employee, and refer to such account number, as well as the employee's name, whenever referencing that employee in any:

(a) letter;

(b) report form;

(c) claim for benefits; or

(d) other communication addressed to the department.
