6.6.3005 REQUIRED FILING DOCUMENTS (1) All filings by insurers which refer to a reference filing of prospective loss costs made by a rating organization shall include, in the order listed, the following documents: (a) reference filing adoption form as specified by the commissioner; and (b) a summary of supporting information.
