2.43.511    FAILURE TO RESPOND -- SECOND NOTICE

(1) A member who fails to submit all medical information as required in the notice will be sent a "second notice" by certified mail, return receipt requested. The second notice will inform the member of:

(a) any specific medical tests or diagnosis required by the board for the review; and

(b) the date on which disability benefits will be suspended if the member does not provide the medical evidence.

(2) The member may request an extension to accommodate scheduled appointments. The written request justifying the need for additional time must be received by the MPERA at least 15 days prior to the end of the time period. Any requests for extensions in excess of 30 days will not be approved.
